As its vision of providing personalized care and support to families and caregivers across the US took off with meteoric success, Careforth was confronted with a technological reality — its legacy system could no longer keep up with ever-growing data volumes. Vention was chosen as a strategic partner to migrate existing processes and business-critical integrations to a more scalable and robust tool.
Our team started by synchronizing data between all third-party APIs to give care coordinators a consolidated view of critical patient records. Next, we introduced an easy-to-use interface built on Lighting UI to minimize clicks and scrolling, while allowing caretakers to spend more time on direct patient care. In tandem with frontend improvements, we automated Careforth’s operational processes like billing, case assignment, and insurance management.
To boost stakeholders’ productivity, we also developed a custom view component to display reports on the status of daily medical exams, as well as added functionality that automatically converts PDF documents into a Salesforce-compatible format.
Our solution seamlessly connected care teams to a 360-degree view of every patient, ultimately improving patient engagement, healthcare outcomes, and communication between all stakeholders.
